Job Title: ACTIVITY AID
Location: SALT LAKE CITY, UT, 84104
Description: Conducts daily activities as assigned by program coordinator. Assists with ambulating, toileting, dressing and feeding of clients as needed. Must demonstrate strong abilities to relate to all clients in a positive manner, with good interaction and communication skills. Preference given for a CNA. Pay Rate $8.50 to $10.00 depending on experience. Schedule Monday through Friday 9:30 am to 5:30 pmResponsibilities:
•Demonstrate emotional and mental maturity necessary for establishing and maintaining good work relationship with clients, client caregivers and personnel of agency
•Maintain involvement and interaction with clients, encouraging client participation in all activities, as needed
•Assist Clients  with ambulation needs, including on & off vans, toileting, personal hygiene, dressing, feeding, walking, toileting, meals and other ADL’s as needed for both male and female clients
•Assist Program Director with materials and set up of daily activities, including involvement and interaction with clients in each activity
•Setup various rooms for specific activity, setup chairs, includes take down and cleaning of rooms after each activity. Leaving room configuration the same
•Maintain and provide clear records, including client charts, on a daily basis and as assigned
•Maintain compliance with all guidelines pertaining to confidentiality of clients
•Assist with driving van to clients homes and from activities on a rotating basis
•Know proper procedures for securing wheelchair-bound clients
•Obey traffic laws
•Practice strict observance of all client rights, including confidentiality
•Administer emergency first aid when necessary
•maintain professional appearance and adherence to dress code
•Maintain in house training requirementsRequirements:
•At least 18 years of age
•A high school graduate or equivalence
•A current, valid state of Utah driver license
•Clean Department of motor vehicles record & signed form giving NHH permission to run a a motor vehicle report
•Ability to drive a 12 to 15 passenger van
•Demonstrate the ability to physically assist clients with ambulating, toileting, dressing, lifting more than 50 lbs or more feeding and other needs as assigned
•TB test before hire
•Current CPR and First AID certification
•Have Salt Lake County Food Handlers Permit
•Communicate effectively in English and Spanish speaking a plus
•Have background check (BCI) done prior to hiring and Drug Free
•Demonstrate desire to care for people, especially those who are elderly and chronically ill and who are incapable of  living independently
